Description
A stapled document printed in black on both sides of paper, with a large folded diagram included at the back of the report. There are 21 printed pages. The report is the result of an Inquiry into the accident which took place near Charing Cross station on the electrified District Line at about 9.55 a.m. on 17th May 1938. The folded diagram at the back includes a track diagram that extends between Charing Cross and Temple stations and beyond both; also, a desciption of the Signalling System and a diagram of a correctly wired No.9 circuit breaker in cabin E.H.

    Shortly after leaving Charing Cross an east-bound Circle train (No. 59) / collided with the rear of an Ealing-Barking train (No. 21) which was standing / at a signal in the tunnel about midway between Charing Cross and Temple / stations; the speed of the Circle train was about 20 miles an hour. Telescoping / took place between two cars at the rear of the Barking train; the rearmost / mounted the underframe of the car in front of it, penetrating its body for about / 15 ft., and rising till its top was in contact with the tunnel roof girders ....
    I regret to report that six passengers were killed, or died in / hospital shortly after the accident, while 43 passengers and two of the trainmen / received injuries, or complained of shock at the time; one of the Board's officers / received electrical burns shortly after the accident. ...
    The accident came about through a "false-clear" signal indication, rendered / possible by a wrong connexion made during the previous night, in the course of / minor alterations to wiring.
